Laparoscopic Cholecystectomy (2)
21 yr old female with congenital kyphoscoliosis and Para paresis- for Lap Cholecystectomy.
Restrictive lung disease because of kyphoscoliosis can cause respiratory distress postoperatively. Such patients often need postoperative mechanical ventilation.
We subjected the patient pre operatively to pulmonary function tests, which showed normal results. Arterial blood gases were also normal.
The patient was also having Para paresis, which can lead to hyperkalemia. So the choice of muscle relaxant at the time of intubation changed from succinyl choline to Atracurium.
The procedure took 45 min and at the end of surgery, patient showed adequate respiratory efforts and was extubated on table. She was having oxygen saturation above 95% on 02 3 lit/min. throughout the first postoperative day. Postoperative pain was taken care by Inj Diclofenac sodium. She was shifted to ward on the third day.
